Our services are completely customizable and vary from client to client. There is no minimum or maximum amount of time we provide care; we can begin work as soon as we are needed, and we work with families, guardians, and care facilities to ensure we meet our clients’ needs.
Margot Nijsure, MA, NHA
Montclair Elder Services owner and director Margot Nijsure, MA, NHA, began her career in nursing home administration throughout the Bay Area. With a masters degree in gerontology from San Francisco State University, and as a licensed nursing home administrator, she spent 17 years in the long-term care field, amassing an extensive book of contacts and a wealth of historical knowledge of the Bay Area’s geriatric care providers and facilities.
In 2020, Margot established Montclair Elder Services to provide authoritative, white-glove geriatric care management in the San Francisco East Bay Area. Her broad depth of experience working throughout the long-term care field allows families to make informed decisions — be those about hospitals, medical centers, home health care, assisted living facilities, or hospice. For families seeking a personal, bespoke response to their elder loved one’s needs, the company provides guardianship, fiduciary and specialized care management assistance. Montclair Elder Services delivers trusted, preeminent care management, companionship and specialized concierge services to seniors, their families and various partner institutions throughout the East Bay Area.

Consultation
We begin the process by providing a complimentary consultation with our lead care manager, who will work with the client and their family or guardian to assess the client’s needs.
Plan of Action
Based on our assessment, we build a comprehensive care plan that includes coordinating wrap-around services with the client’s Care Team.
Care Management
We pride ourselves on facilitating continuous, fluid communication between everyone on the Care Team, ensuring exceptional, seamless care is provided to our client.
Dementia Care Support
Dementia care from Montclair Elder Services assists individuals with early-stage, mid-stage, or late-stage dementia. These services provide support to clients with conditions such as Alzheimer’s disease, vascular dementia, Lewy body dementia, or Huntington’s disease. Understanding that each form of dementia has a different cause and rate of progression, we create a distinctive care plan tailored to your loved one’s condition to make daily life as comfortable and enjoyable as possible.
Our dementia care services extend to family members offering care to dementia patients. If you act as a caregiver for a loved one, Montclair Elder Services assists in making caregiving more manageable and less overwhelming. As a result, you can dedicate more time to self-care, other family obligations, or simply spending quality time with your loved one.
Home Safety Evaluation
Montclair Elder Services specializes in evaluation of a client’s home to identify and eliminate potential hazards. The home safety assessment may also include home improvement recommendations to help your loved one navigate their environment more securely, such as installing handrails in the bathroom or making sure the home has adequate lighting.
According to the National Council on Aging, one in four Americans age 65 and older falls every year, and falls are the number one cause of injury-related death for older adults. Our evaluation is an invaluable preventive intervention for your loved one’s continued independence and your peace of mind.
Healthcare Advocate
We are pleased to accompany clients to medical visits, ensuring they get to their appointments safely and on time. Montclair Elder Services provides door-through-door services, meaning that unlike a transportation service, we are companions for medical visits, present every step of the way. We assist clients in and out of the building and help with any necessary paperwork during check-in and check-out. If appropriate, we remain present during the consultation so as to act as an extra set of eyes and ears, helping your loved one remember and ensuring follow-up care instructions are clearly understood and implemented. Following the appointment, we can also take care of any prescription needs with the client’s local pharmacy.
Our services allow for less stress and confusion for the client and family, and result in better outcomes for the health of the client.
Hospital-to-Home Support
An individual who has been hospitalized is often apprehensive to be transferred to a new place, even if that place is his or her own home.
Montclair Elder Services offers a full range of non-medical services for seniors in transition:
- We accompany our clients to their new setting, and make sure they are settled.
- We visit them to assure a positive transition.
- We fully outfit the home for a client’s return, including the acquisition of needed medical equipment and the securing of meal services.
- On the way home, we pack up belongings, fill out discharge papers, run any errands, accompany them home and make sure they are comfortable and settled.
- We keep families updated on the status and well-being of their loved ones at all times.

We are well-educated professionals with a passion for the clients we serve. We represent decades of experience in the aging sector, and take pride in assessing the unique needs of each client to provide the highest caliber experience.
We do not require a minimum number of hours for our services, and will customize a bespoke service plan based on your needs.
